Office Policies

  • Patients who fail to show up for their scheduled appointment without giving the office at least 24-hour’s notice in advance will be considered a “No-Show” for the visit and subject to a no-show fee.  Repeated no-shows may result in being dismissed from the practice. This includes patients calling to reschedule a visit with less than 24-hour’s notice.

  • We understand things happen and there may be a time when you are running late for a visit with us. We ask that you call our office to let us know that you are running late. Patients arriving after their scheduled visit time may not be seen and will need to reschedule their visit depending on the provider’s discretion and availability.

  • All lab results are posted to the patient portal within 24-hours of being received and reviewed in our office. If you wish to discuss your lab results you will need to schedule a visit with your provider. Lab result visits may be scheduled in person or by telemed.

    All NaProTECHNOLOGY consultations will require a follow up visit with your provider to review the results and determine the next step of your care.  

  • We will not prescribe controlled substance refills on the first visit.  In order to be prescribed a controlled substance in this practice, we must first obtain records from your previous doctor(s).  The patient will also have to sign a controlled substance agreement and an INspect report will be obtained (a pharmacy report of all previously prescribed controlled substances).  In addition, a urine drug screen may be required.

  • Most insurance companies do not cover school or sports physicals outside of an annual wellness exam. If you need a physical completed outside of a wellness visit, we charge a flat rate of $85 (self-pay only) for such a visit.

  • Completion of medical forms such as disability, life insurance applications, FMLA, etc. are $25 per form to be completed. An office visit may be required before any forms can be completed.

  •  We welcome patients without insurance and offer a discounted fee schedule for visits. Self-pay patients will be required to pay an $85 deposit at the time of check-in and this amount will be applied to the total charge for the visit. We can provide limited price estimates, however final billing is determined by the medical coding the provider uses after each visit is completed and may not be fully known in advance.

  • Patient are expected to pay any outstanding balances promptly, including any prior balances at the time of check-in for a visit. Patients with larger balances may apply for a payment plan. Patients with outstanding balances and no payment plan may be subject to being sent to collections or being dismissed from the practice.

  • Crieghton starter kit = $60

  • ‍ The patient portal (health.healow.com/magnificat or the Healowapp) is the preferred method of communication between our office and patients. The portal can be used to request or change an appointment, view/pay your bill, and ask general questions, etc. Parents of minors or caregivers may request proxy access to patient portals for those in their care.

    ‍Certain medical advice messages sent through the patient portal may be billed as an "e-visit" and submitted to your insurance or billed to you directly. This applies to messages that require clinical evaluation and management from a provider (physician or physician assistant), take more than five minutes to assess and respond, and fall within the following parameters:

    • The service is initiated by an established patient.

    • The service requires the medical provider’s professional medical decision making.

    • The service is unrelated to a visit that occurred within the prior seven days.

    • No other visit (in person or telemed) occurs within seven days after the portal communication service is provided.

    ‍Examples of portal communications that MAY be billed:

    • A new health issue or symptom requiring a provider's assessment.

    • An update to an existing problem that requires a change in your treatment plan.

    • Adjustments to medication or dosage.

    • Requests for new prescriptions for a chronic condition.

    ‍Examples of portal communications that will NOT be billed:

    • Scheduling or canceling appointments, including information about referrals.

    • Prescription refill requests (of a non-controlled substance).

    • Quick questions answered by staff regarding previously discussed issues.

    • Technical support questions regarding the portal.

    • Requesting copies of your medical records.

    • Questions about insurance or billing

  • We follow national Medicare/Medicaid and American Medical Association coding guidelines. These guidelines mandate that we code and bill for services in a specific manner.  A wellness exam is a preventative service that may include a routine physical exam and age-appropriate tests. A wellness visit does not include any new or chronic health problems discussed, addressed, or treated during the visit. For example, if during your wellness exam the provider becomes aware of issues such as infections, pre-existing conditions, complaints of pain, rashes, etc., and believes additional medical evaluation or services are necessary to address these other concerns an additional office visit code may be assessed.  This means you may be billed for a wellness exam and a problem-focused office visit code for the same appointment.

  • Shared Savings Program – Accountable Care Organizations

    ‍Magnificat Family Medicine participates in the Palm Beach Accountable Care Organization (PBACO) to provide coordinated care for our Medicare patients. Representatives from PBACO may contact you to assist us with scheduling annual wellness exams or follow up appointments in our office.

    ‍Medicare protects the privacy of your health information. If you don’t want Medicare to share your health information with your doctors for care coordination, call 1-800-MEDICARE (1-800-633-4227). Medicare may still share general information to measure provider quality. For more information on how Medicare may use and give out your information, visit Medicare.gov and search for “privacy.”

    Remote Patient Monitoring Available

    ‍Magnificat Family Medicine has partnered with Synsormed, a trusted healthcare technology company, to enhance patient support, communication, and continuity of care for our Medicare patients with chronic health conditions. Synsormed will provide home monitoring devices (such as blood pressure monitors) that report findings back to our office. Synsormed works securely alongside our clinical team to help deliver timely check-ins, reminders, and educational support related to your care plan. Any outreach you receive through Synsormed is legitimate, confidential, and directly connected to your care at Magnificat.
